Admission Process
For planned or scheduled admission, please take note of the following simple reminders:
- Prepare your identification cards indicating personal information, such as your current address, phone number and the likes. It is best to bring a valid government-issued photo ID. Below are common ID cards:
- Accredited HMO-issued member’s ID
- Senior Citizen’s ID/ Persons with Disability ID
- PhilHealth ID
- Healthcare insurance card or document
- Digitized SSS ID
- GSIS ID
- PRC ID
- Unified Multi-Purpose ID
- Driver’s License
- Passport
- National ID
- Bring any relevant health information documents such as test results or a list of your current medications (prescription, over-the-counter, and herbal).
- Plan for your payment arrangements. Bring your healthcare cards or insurance coverage forms, as well as any special form(s) or pre-authorization documents such as Letter of Guarantee (LOG) or Letter of Authority (LOA).
